At Vista Eye Associates, we are committed to providing you with the best possible service and quality when it comes to your frames and lenses. Please read the policies below, so that you are familiar with all of your options.
Payment is expected at the time professional services are rendered and no refunds will be made for services. Due to the custom nature of glasses, all prescription eyeglasses and sunglasses are final. No refunds will be given.
We will contact your insurance company to determine your benefits before you arrive for your appointment. You will be responsible for any out-of-pocket cost your insurance does not cover, which includes co-pays, deductibles, co-insurances, and any out of pocket costs on eyeglasses and contacts.
Eyeglasses Prescription Rechecks:In the event you are having trouble with your new prescription eyeglasses purchased at Vista Eye Associates, an eyeglass recheck may be performed. If a new prescription or lenses are necessary, a courtesy one time re-make will be provided at no charge within 90 days of the exam. Re-check visits and/or lenses after 90 days will be charged the usual fees.
Frame Warranty:Frames purchased at Vista Eye Associates have a one time, within one year manufacturer defect warranty. This will not cover loss or theft. We reserve the right to inspect and determine if the frame can be replaced under warranty.
Scratch Warranty:Lenses purchased with an Anti-Reflective coating have a 1 year scratch warranty. The number of times this warranty can be used within a year is dependent on the type of Anti-Reflective coating that was purchased. Please consult with your eyewear specialist for the details. Lenses purchased without an Anti-Reflective coating do not have a warranty against scratches. We are not responsible for eyeglasses not picked up within 90 days. Payments or deposits will not be refunded. Any other unusual circumstances that are not mentioned above will be determined on a case-by-case basis.
APPOINTMENT CANCELLATION/NO SHOW POLICY
• Effective February 1, 2023- any patient who fails to show up to their appointment or cancels/reschedules an appointment without giving our office at least a 48 hour notice will be considered a No-Show and charged a $25.00 fee.
• The fee is charged to the patient, not the insurance company, and is due before we can reschedule the patient’s next visit.
• As a courtesy, when time permits, we make reminder calls, send out text messages and/or emails for your appointments. Even if you do not receive a reminder call or message, the above policy will remain in effect.
• After a third no-show or cancellation/reschedule without at least a 48 hour notice, the patient may be dismissed from Vista Eye Associates.
